  • The required documents for a DOT physical appointment typically include:

    • Valid photot id

    • Complete list of ALL medications( inc the doses and Dr’s names and addresses)

    • medical records related to existing conditions

    • corrective lenses or hearing aids if needed

    • If you have diabetes your blood sugar must be controlled ( Bring the most recent results of a lab test A1C and your blood sugar logs

    • Drivers that have sleep apnea bring a reading from your machine documenting your proper use of the machine, letter from your specialist may also be required ( bring at least 90 days of data from past year)

    • Letter from your primary physician or specialist if have any medical conditions

    • Drivers with heart related issues ( at minimum , bring a letter from your cardiologist that outlines your medical history and current meds and indicates your are safe to drive a DOT vehicle. You may also need to bring the results of a recent stress test, ECHO, or other testing completed within the past 1-2 years.

    • Drivers who have suffered a stroke, brain tumor,seizure or bleeding of the brain bring a letter from your neurologist and nerve specialist that outlines your medical history, current meds, and current neurologic and psychiatric state.

    • Drivers taking any meds that may cause sedation or sleepiness or controlled substances( narcotics, anxiety meds, sleeping pills, ADHD meds will need a note and medical records from your treating physician regarding the safety of driving a DOT vehicle while using these meds.

    • Drivers who take blood thinners bring a recent INR blood level and clearance letter from your doctor.

    • medical waiver or exemption documentation if applicable

  • Some health conditions have exemptions, and leaving something out could lead to failing the exam and having your CDL license revoked.
